Obituary for Nick Kozak
Nicholas James Kozak, age 42, passed away unexpectedly at his home in Peru on March 31. Cremation rites have been accorded.
Nick was born on April 17, 1978 at St. Margaret’s Hospital in Spring Valley to James and Jeanie (Jakubiak) Kozak. He was a loving father to Jaxon; son of James and Jeanie (Jakubiak) Kozak; brother of Douglas (Tanya) Kozak and Joseph (Gina) Kozak; cherished uncle of Vera, Vivian, and Rocco; and dear friend to many.
Nick was a dedicated forklift operator at XPO Logistics in LaSalle. He enjoyed spending time with his family and friends. Most importantly, he loved spending time with his son Jaxon and was a proud father in attendance at all his sporting events. Win or lose, Nick was an avid Bears and Cubs fan.
He is preceded in death by his paternal and maternal grandparents and sister Sara (1980).
A memorial visitation will be held at St. Valentine’s Church on Monday, April 5 from 9am until the time of services. (Masks are required). A memorial mass will be held in the church at 11am. Father Small of Peru Catholic Parishes will officiate. Burial will follow at St. Joseph’s Cemetery.
The Burgess Funeral Home in LaSalle is handling the arrangements.
In lieu of flowers memorials may be sent to his family for his favorite charities.
